What this means for your family

While the facility is physically beautiful and offers excellent social activities, there is a recurring pattern of concern regarding clinical oversight and rehab effectiveness. When touring, we strongly recommend asking for specific examples of how they manage fall risks and dietary restrictions, as these have been cited as major failures by multiple families.

Key Review Excerpts

My mom was here for end of life care and the care and dedication she received was amazing. Abby the admissions girl made the admission stress free, the social worker Tammy always checked in with us, the BOM Mandy did a an amazing job helping us understand insurances.

Memory care family member · 2024★★★★★

My father had to ask for bed rails to be put on his bed. Only give 30 minutes of rehab 5 days a week and they only have a few pieces of equipment for the patients to use for rehab. My father complained that the food was worse than road kill.

Rehab patient's family · 2024☆☆☆☆

They admitted my mom knowing she had Alzheimer’s disease but the staff had no idea how to care for her. Their lack of knowledge was dangerous.

Clean record — no deficiencies

One family filed a complaint that led to a deficiency for treatment and care in late 2024. The facility's main recurring issues involve medication management, electrical safety systems, and resident activities. While all deficiencies have been corrected according to the provider, the pattern shows ongoing challenges with medication safety across multiple surveys, suggesting families should inquire about current pharmacy practices and safety protocols.
