The most recent inspection from March 2025 identified several concerns regarding medication administration accuracy, incomplete meal records, and failure to properly log dishwasher temperatures. Some of these issues regarding meal records and kitchen sanitation logs were noted as repeat violations.

Medication administration errors: Residents had multiple PRN medications for the same purpose without clear instructions on usage, and insulin was documented as administered even when blood sugar levels were below the required sliding scale threshold. Some MAR entries were also left blank.
Repeat violation: Meal census and production logs were left blank on several dates in January and February 2025.
Repeat violation: Dishwasher temperature logs were incomplete, with missing temperatures or initials on numerous dates throughout January and February 2025.
