The following deficiencies were found during the annual compliance inspection conducted on July 29, 2025:

Based on record review, documentation review and interview, the administrator failed to ensure the health care institution developed and administered a training program for all staff regarding fall prevention and fall recovery. The deficient practice posed a risk to the physical health and safety of a resident. Findings include: 1 . A review of E1's, E2's and E3's employee records revealed no training for Fall Prevention and Fall Recovery. 2 . A review of facility documentation revealed no policy and procedure to address the Fall Prevention and Fall Recovery Program training. 3 . In an interview, E4 reported that E4 was in the process of reviewing the facility policies and procedures and rewriting several of the polices. E4 acknowledged that there was no Fall Prevention and Fall Recovery training set up for the employees.

Based on record review and interview the administrator failed to ensure that documentation was obtained for each individual required to be screened for infectious TB that indicated the individual's freedom from symptoms of infectious TB and signed by a medical practitioner, occupation health provider, as defined in A.A.C. R9-6-801, or local health agency, as defined in A.A.C. R9-6-101. Findings include: 1 . A review of E1's, E2' and E3's personnel records revealed no Tuberculosis signs and symptoms screening to indicate that the employees were free from symptoms of infectious tuberculosis. 2 . In an interview, E1 acknowledged that there were no tuberculosis signs and symptoms screening forms for the employees available for review.
