What this means for your family

While the facility's rehabilitation team is highly effective for post-injury recovery, the serious allegations regarding medication security and staff professionalism are concerning. We strongly recommend that families conduct an unannounced visit and inquire specifically about medication administration protocols and staff-to-patient ratios before choosing this location.

Questions for Your Tour

  • 1Given that physical therapy is a noted strength here, how do you integrate those services into a resident's daily routine to ensure they stay active?
  • 2I noticed the staffing rating is a bit lower than the clinical ratings; could you walk me through how you manage daily staffing levels to ensure consistent care for all 60 residents?
  • 3Communication is very important to our family; what is your standard process for updating family members on changes in a resident's health or medication adjustments?
  • 4Since high-quality food is a highlight of your facility, how do you handle dietary preferences or special nutritional needs for residents?
  • 5What protocols do you have in place for medication management to ensure accuracy and timely administration for your residents?
  • 6How do you foster a sense of community and social engagement for residents during their downtime outside of scheduled therapy or meals?

Key Review Excerpts

I spent a few months there after cracking my hip last year. I found the staff very friendly, a great cook, and the therapists were great. They got me back on my feet almost as soon as I got there.

Rehab patient · 2025★★★★

Nursing staff care more about spreading gossip and rumors than about their patients wellbeing and health. The DON is a joke and chastises & demeans staff in front of anyone & everyone. This facility is very understaffed imo.

Observer/Family member · 2025★★☆☆☆

My Mother was only there for 3 1/2 days before I took her out. During that short time her medication was stolen and replaced with water.

Clean record — no deficiencies

Families filed complaints that led to findings about medication errors and failure to notify families of resident incidents, though both issues were reportedly corrected. The facility has recurring problems with fire safety systems, electrical equipment, and emergency preparedness across all three surveys, suggesting ongoing building maintenance challenges. All deficiencies show correction dates, but the pattern of repeated fire safety violations raises concerns about sustained compliance.
