The following deficiencies were found during the on-site compliance inspection conducted on August 5, 2025:

Based on observation and interview, the administrator did not ensure the premises and it's structures and furnishings were in a clean condition. Findings include: 1 . During an environmental inspection, the Compliance Officer observed the following furnishings not in a clean condition: The fans in the home appeared to be unclean and contained dust build-up; and The air intake for the facility's ventilation system appeared unclean. 2 . In an interview, E6 acknowledged the furnishings were not in a clean condition.

Findings include: 1 . During an environmental inspection, the Compliance Officer observed two toilets in resident bathrooms. The toilets appeared to have shifted and the caulking surrounding the toilets appeared to be damaged and detached. 2 . In an interview, E6 acknowledged the premises furnishings were not in sufficiently good repair.

Based on review of facility documentation, the facility failed to ensure disaster drills for employees were conducted on each shift at least once every three months and documented when completed. Findings include: Review of the facility's evacuation and disaster drill for 2023 revealed that there were evacuation drills conducted every month however, no disaster drills were documented as completed. The policy on Evacuation/Relocation Procedures revealed that the staff and members will conduct a fire and evacuation drill three times each quarter at various times of the day. However, the policy did not include conducting disaster drills and when the disaster drills should be conducted.
