Key Review Excerpts

The only good thing about this place is the therapy unit and the therapist that work in the therapy unit……

Long-term resident's family · 2025☆☆☆☆

He was always drugged up to the point he no longer wanted to eat. Bed sores and wasn't changed on a regular schedule. Clothes and shoes were all stolen weekly with no explanation.

Hospice family member · 2023☆☆☆☆

My stay for rehab was great. The staff was kind and attentive and the therapy team was exceptional!

8 complaint-triggered

Brookview Healthcare Center shows concerning patterns with 42 deficiencies over six surveys, including five complaint-triggered violations where families reported problems. Recurring issues involve medication management, resident rights, and safety systems. While the facility has corrected most deficiencies, persistent problems with medication errors and fire safety systems across multiple years suggest ongoing operational challenges families should carefully evaluate.
