The Recertification survey was conducted June 24, 2025 through June 26, 2025 in conjunction with the investigation of intake #s SF00131685. AZ00224268, AZ00224171, AZ00224170. SF00124781, and AZ00224994. The following deficiencies were cited:
Based on observation, clinical record review, staff interviews, and facility policy review, the facility failed to ensure that medications were administered, stored and residents were assessed for self-administration of medication for residents (#1).
Based on observation, clinical record review, staff interviews, and facility policy review, the facility failed to ensure that medications were administered, stored and residents were assessed for self-administration of medication for one residents (#1). The deficient practice could result in medications not being administered according to physician's orders and medications not being stored safely.
Based on clinical record reviews, staff interviews, facility documentation, and facility policy, the facility failed to ensure one resident (#332) was free from preventable accidents including oral administration of Dakins solution. The sample size was 13. This deficient practice could result in an adverse event for the resident.
