Five resident refrigerators/freezers did not contain a reliable thermometer.
Multiple food items in various refrigerators and cabinets were unlabeled with appropriate open dates.
No record of a meal census (kind and amount of food used) was maintained for the preceding 3 months.
One resident's TB screening date was out of compliance (not performed within 12 months prior to admission).
Hazardous/toxic chemicals were stored in an unlocked cabinet, and sharp items were found unsecured in the activities area.
Clean linens were being stored on the floor in the upstairs laundry room rather than in a separate storage room.
Dishwasher sanitization logs contained incomplete or blank entries for temperatures between March 20, 2025, and April 4, 2025.
Two employees' TB screenings were not completed within 10 days of hire, and one employee's TB screening could not be found.
