Key Review Excerpts

While in the medical facility, Hallowell, my mother supposedly fell and hit her head in 2 places but staff did not call a doctor, ambulance nor a family member for more than 5 hours.

Rehab patient's family · 2024☆☆☆☆

The staff has frequently not followed the protocol for administering her medications. On a more serious note, this 91 yr old woman has twice been left outside in 90 plus weather. The staff does not respond to the alarm that she carries.

Friend of resident · 2019☆☆☆☆

Mom has lived there independently for several years and now in Assited Living apartment. Beautiful grounds, caring staff, many Cultural activities and excellent food options made it an easy transition to an active senior community.

6deficiencies
Well below state avg (18.9)

Broadmead has 19 federal deficiencies across three surveys, with all violations corrected by the facility. The most recurring issues involve fire safety systems (sprinklers, emergency exits, smoke barriers), medication management and pharmacy services, and care planning. While no families filed complaints, the facility has faced repeated problems with fire safety compliance across multiple years, though pharmacy oversight issues appear to have been resolved after 2019.
