The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. A complaint was received by VDSS Division of Licensing on regarding allegations in the area(s) of: PERSONNEL; STAFFING IN A SAFE, SECURE ENVIRONMENT Number of residents present at the facility at the beginning of the inspection: At the time of the on-site inspection, there were Twenty-Four (24) residing in the safe, secure wing of the facility. Number of interviews conducted with staff: 1 Observations by licensing inspector: While touring the safe, secure wing of the facility, it was noted the required staff to resident ratio was met. The number of staff on duty during the inspection exceeded the required staff to resident ratio. Residents were noted to be appropriately dressed and groomed for the time of day and activities they were engaged in. Some residents were observed eating, while others were interacting with other residents and staff. Interactions were noted to be appropriate and professional. The physical condition of the unit was clean and well-maintained. No safety concerns were identified. Additional Comments/Discussion: In addition to physically witnessing the staff to resident ratio during the on-site review, a review of the staffing schedule for December 2025 and January 2026 were reviewed. The staffing schedules meet the staff to resident ratio requirements. It was also discussed with the administrator the processes that are used when a staff member calls out sick, is late, etc? The evidence gathered during the investigation did not support the allegation(s) of non-compliance with standard(s) or law.
