Key Review Excerpts

The facility is clean but the staff are not very sweet, they do not pay attention to the residents or properly clean the residents from what I have been told by my memaw.

Grandchild of resident · 2024☆☆☆☆

From what I have seen they are under staffed and my wife can't see much and when she asks for something to eat she does not always get it she has diabetes her blood sugar goes up and down a lot.

Spouse of resident · 2023★★☆☆☆

I was then threatened by administration that the police would be called and they would arrest me for trespass and then a restraining order issued!

3deficiencies
Well below state avg (11.3)

Borderview Rehab & Living Center shows a pattern of recurring deficiencies across multiple surveys, with issues persisting in resident rights, food safety, and daily care assistance appearing in recent 2024-2025 inspections. While all violations have correction dates, the facility has struggled with basic care standards, nursing staffing requirements, and infection control policies across three separate surveys. The repeated nature of deficiencies in core care areas suggests ongoing operational challenges that families should carefully consider.
