What this means for your family

While the facility has recently received a high volume of positive reviews, these are heavily contradicted by long-standing, severe reports of neglect and hygiene issues. If you consider this facility, perform an unannounced visit during a weekend or evening shift to observe actual response times and cleanliness, and specifically ask for the facility's most recent state survey results.

Bluebird Wellness and Rehabilitation, formerly known as Green Park, has a long history of severe complaints regarding neglect, poor hygiene, and unprofessional staff. While recent reviews from 2025 show a cluster of positive feedback praising specific staff members and admissions processes, these are heavily contrasted by ongoing reports of residents being left in soiled linens, long response times for call lights, and poor food quality. Families should exercise extreme caution as the facility's reputation remains highly polarized.

Key Review Excerpts

The right hand does not know what the left hand is doing. Upon arrival, things seemed ok, but as the day and night progressed, we learned that we had made the wrong decision in coming here.

Family member of resident · 2026☆☆☆☆

The scent of urine in the halls was strong however, and should be addressed. Entry policy not stated on the door, unclear as to what to do to enter or exit building.

Visitor · 2025★★★★

My dad was in for rehab and they got him up 1 time in 2 weeks. Also, they forgot his meds often and didn't bathe him at all during his stay. Plus, the staff was incredibly rude and constantly yelled and my poor dad.

Families have repeatedly filed reports triggering 54 out of 121 deficiencies, with persistent problems in wound care, medication management, and nursing staffing. Issues with pressure ulcer care have occurred four times in recent months, while medication errors and inadequate staffing continue appearing across multiple complaint investigations. Though the facility reports correcting violations, the recurring nature of these serious care problems, particularly wound care failures, raises significant concerns about sustained quality improvement.
