The facility failed to either assess a resident or to develop, approve, sign, or follow a service plan within the allowable time.

The facility failed to obtain and maintain a properly completed Request for Authorized Electronic Monitoring in the clinical record.

The facility failed to procure food from acceptable sources, or failed to handle food, subject to spoilage, as required.

The facility failed to ensure the fire alarm was always visible to staff or is monitored by a device always carried by the staff.

The facility failed to ensure a written emergency plan that addresses the eight core functions of emergency management was provided.

The facility failed to ensure lighting levels were in compliance with licensing standards for assisted living facilities and the Illumination Engineering Society of North America.

The facility failed to ensure the building and structure complied with the 2000 edition of the Life Safety Code, NFPA 101.

The facility failed to provide an initial gas pressure test of the gas lines from the meter; and/or failed to ensure additional gas pressure tests were performed when gas service was interrupted; and/or that all gas heating systems were documented

The facility failed to ensure compliance with ADA, TAS, and TDLR standards; and/or failed to provide evidence of plan submittal to the Elimination of Architectural Barriers Program of the TDLR.
