The following deficiencies were found during the on-site compliance inspection conducted on September 11, 2024:

When medication is stored by an assisted living facility, a manager shall ensure that:R9-10-816.F.1

Based on observation and interview, the manager failed to ensure medication stored by an assisted living facility was stored in a separate locked room, closet, cabinet, or self-contained unit. The deficient practice posed a risk to residents. Findings Include: 1. The Compliance Officer observed an unlocked medication cabinet in the facility's kitchen that contained the following medications: - Baclofen 10mg - Tramadol 50mg - Acetaminophen 325mg 2. The Compliance Officer observed Nystatin 100000U/GM unlocked and unsecured in R1's bathroom. 3. In an interview, E1 confirmed that the medicine cabinet in the kitchen was unlocked and that Nystatin was stored by the facility unlocked on R1's bathroom counter.

Based on observation and interview, the manager failed to ensure that one of three employees had two-step tuberculosis screening. The deficient practice posed a risk to residents. Findings include: 1. The Compliance Officer observed that E3 (date of hire July 3, 2024) did not have a first step or second step tuberculosis test. 2. In an interview, E1 reported that E3 did not have a first step or second step tuberculosis test.

A manager shall ensure that a resident's medical record contains:R9-10-811.C.13.a

Based on observation and interview, the manager failed to ensure medication administration was properly documented at the time that the medication was administered. The deficient practice posed a risk to residents if it could not be determined if the medications were actually administered, and the Department was provided false and misleading information. Findings Include: 1. The Compliance Officer observed R1's medication administration record completed for the entire day of September 11, 2024, including bedtime medications. September 11, 2024 was the date of the compliance inspection and this observation occurred in the morning hours. 2. In an interview, E1 confirmed that R1's medication administration record was completely filled out for September 11, 2024.
