A complaint was received by VDSS Division of Licensing on February 12, 2026, regarding allegations in the area(s) of: RESIDENT CARE AND RELATED SERVICES Number of interviews conducted with staff: 2 Additional Comments/Discussion: Documentation of resident #1?s injuries were reviewed as part of the investigation. An exit meeting will be conducted to review the inspection findings. The evidence gathered during the investigation supported the allegation(s) of non-compliance with standard(s) or law, and violation(s) were issued. Any violation(s) not related to the complaint(s) but identified during the course of the investigation can also be found on the violation notice. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law. If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview. Based on a record review and interviews, it was determined that the facility did not ensure the safety and well-being of a resident was maintained. Evidence: 1) On 12/30/25, resident #1 was noted to have bruising on his chest that could not be explained. 2) On 12/31/25, resident #1 complained of pain on his trunk/side and staff noted that resident #1 had bruising on their left knee, left shoulder, right ribcage, and a bump on their forehead. 3) Staff #1 and staff #2 stated that staff #3 (former employee) reported that resident #1 fell in a communal bathroom on 12/30/25. Staff #3 described resident #1 as being in a sitting position on the trash can/floor. 4) Staff #1 and staff #2 were unable to explain with certainty how resident #1 was injured.

Interactions were appropriate in all instances observed. Residents were observed eating meals and engaging in staff led and self-led activities. Evidence: Resident #3: Documented date of admission 03/16/2022 The facility?s 05/24/2022 dietician review report referring to the facility and the resident?s dietary intake noted ?They note a decrease of intake.? The report further notes that the resident said that he wanted to lose weight. Upon request the facility did not submit documentation for the inspector?s review that identified a plan of care based on the resident?s preferences or that was guided by the resident?s physician.
