What this means for your family

While the facility is physically clean and features a strong therapy department, the recurring reports of neglect regarding fall-risk patients and medication errors are significant red flags. If you choose this facility, ensure you have a plan for frequent, unannounced visits to monitor your loved one's care and maintain direct communication with the nursing supervisors.

Berlin Rehabilitation and Healthcare Center receives highly polarized feedback, with many families praising the facility's cleanliness, welcoming front desk staff, and effective physical therapy team. However, there are serious, recurring allegations of neglect, including reports of medication errors, failure to monitor high-fall-risk patients, and poor communication regarding medical status. Families should be aware that while many visitors have positive experiences, there is a significant pattern of complaints regarding staff responsiveness and patient safety.

Key Review Excerpts

The staff especially front desk are fabulous!!! The Dr sent dad to hospital today for an issue he was having ….the issue was that staff, or maybe not, think that trained staff would know what way a catheter goes in. My dads was upside down inside of him and positioned wrong of coarse.
