What this means for your family

While this facility has a standout activities department and highly skilled wound care nurses, the recurring reports of neglect and hygiene issues are deeply concerning. If you consider this facility, we strongly recommend unannounced visits during weekends and evenings to verify staffing levels and the quality of basic patient care.

Key Review Excerpts

My dad Mr Harlod Cummings has been at Azalea for 6 months. His CNA Tina Miller is amazing she go above and beyond for my dad

Long-term resident's family · 2024★★★★★

Beverly Fuller and Quemecka Callaway are the best wound care nurses I have ever had. I came from Florida and have been treated at other places in Augusta and these two ladies were able to get my wounds healed.

Rehab patient · 2024★★★★★

The Facility has a great staff on days and night. The down side is the staff minimal required personnel which means they are over worked. The food is a mystery ( unseasoned unidentifiable mush plates) every meal unless you request a sandwich.
