The following deficiencies were found during the on-site compliance inspection conducted on May 23, 2023:

A manager shall ensure that:R9-10-818.A.4Corrected Jun 15, 2023

Based on documentation review and interview, the manager failed to ensure a disaster drill for employees was conducted on each shift at least once every three months and documented. Findings include: 1. A review of facility documentation revealed a staffing schedule with the following two shifts: -1st Shift, 7:00 AM-7:00 PM; and -2nd Shift, 7:00 PM-7:00 AM. 2. A review of facility documentation revealed documented disaster drills for the following dates and times: -October 10, 2022 at 11:00 AM and 8:00 PM; and -May 2, 2023 at 3:00 PM. However, no additional documentation of disaster drills were available for review. 3. In an interview, E4 reported E4 was just hired at the beginning of the month to help manage the facility's administration. E4 acknowledged disaster drills were not conducted on each shift at least once every three months.

Based on observation and interview, the manager failed to ensure hot water temperatures were maintained between 95\'b0 F and 120\'b0 F in the areas of a facility used by residents. Findings include: 1. Using a Department-issued thermometer, the Compliance Officer observed the hot water temperature measured 122.4\'b0 F at the sink of a common bathroom used by residents. 2. Using a Department-issued thermometer, the Compliance Officer observed the hot water temperature measured 121.4\'b0 F at the sink of a second common bathroom used by residents. 3. In an interview, E2 acknowledged hot water temperatures were not maintained between 95\'b0 F and 120\'b0 F.

Based on observation and interview, the manager failed to ensure poisonous or toxic materials were stored in a locked area and inaccessible to residents. Findings include: 1. The Compliance Officer observed three ambulatory residents on the premises. 2. During the environmental inspection of the facility, the Compliance Officer observed a spray can of "Oxi-Clean Foam-tastic" bathroom cleaner, a spray bottle of "Clorox Cleaner + Bleach", a jug of "Clorox" liquid bleach, three jugs of laundry detergent, a bottle of "Pine Sol" multi-surface cleaner, and a jug of "Ajax Ultra" liquid dish soap in an unlocked cabinet in the facility's unlocked laundry room. The cabinet door had a lock, however the door was unlocked at the time of the inspection. E2 locked the cabinet door while the Compliance Officer was on site. 3. In an interview, E2 acknowledged poisonous or toxic materials stored by the facility were not stored in a locked area.
