Key Review Excerpts

Some days, they forgot to give him breakfast and forgot to get him something to drink. Wreaked of cigarette smoke like someone was smoking inside. Shared bathrooms and someone bled in the bathroom the night before and was still in there the next day along with blood on the hallway floor.

12 complaint-triggered

This facility shows concerning patterns with multiple family complaints triggering inspections, including a recent abuse/neglect violation and repeated safety hazards. Key problem areas include accident prevention and safety supervision (cited multiple times), care planning, and nursing staff oversight. While all deficiencies show correction dates, the recurring nature of safety issues and the fact that families have filed eight complaint-triggered reports suggest ongoing care quality concerns that families should carefully consider.
