What this means for your family

This facility has a capable therapy team, but the recurring reports of neglect, poor hygiene, and communication failures are serious red flags. If you choose this facility, you must be prepared to be a daily, active advocate for your loved one, as the facility is frequently understaffed and unresponsive to family inquiries.

Avamere Rehabilitation of Hillsboro receives highly polarized feedback, with some patients praising the physical and occupational therapy teams, while others report severe lapses in basic care and communication. Families frequently express frustration over the facility's aging infrastructure, poor responsiveness to call lights, and difficulty reaching staff for updates. While some individuals report excellent recovery experiences, the recurring reports of neglect and understaffing make this a high-risk choice for those requiring intensive oversight.

Questions for Your Tour

  • 1Given the facility's focus on rehabilitation, what specific protocols are in place to ensure consistent communication between the therapy teams and family members regarding progress?
  • 2I noticed the facility has a high volume of therapy success stories; how do you ensure that same level of dedicated attention is maintained for daily personal hygiene and routine care needs?
  • 3Could you walk me through your current staffing plan during evening and weekend hours to ensure that residents receive timely assistance when they use their call lights?
  • 4I see that you actively engage with online feedback; how has that family input specifically influenced recent changes to your dining program or food quality?
  • 5With the building being an older facility, what is your current plan for ongoing maintenance and ensuring that resident rooms remain clean and comfortable?
  • 6In the event of a medical change or emergency, what is your standard process for notifying family members to ensure we are kept in the loop immediately?

Key Review Excerpts

The facility is little misleading at first sight, it could use a coat of fresh paint and some wall patching on the inside. That said, I would still rate as five stars because of the medical professionals and staff that took care of me.

Rehab patient · 2025★★★★★

Took 45 mins. Or more to get any help after ringing the bell for emergencies. Covid was rampant. We had to get tested almost every day. The facilities were not clean, especially the bathroom. They were clearly shortstaffed.

Rehab patient · 2025★★☆☆☆

The staff don't seem to care about the patients. My mother stayed here after femur surgery and her care was so poor, she ended up back in the hospital with several infections, including staph.

This facility shows concerning patterns with families filing formal complaints, leading to 5 complaint-triggered inspections over recent years. The main recurring issues involve resident safety and accident prevention, resident rights violations, and treatment/care quality problems. While most deficiencies appear corrected after discovery, the pattern of safety hazards and families feeling compelled to file formal reports about care quality suggests ongoing supervision and care coordination challenges that prospective families should investigate thoroughly during visits.
