Key Review Excerpts

My mother entered the facility for sub, acute rehab in April 2023. The care was great. The staff were great. The rooms were spacious and clean. When my mom transferred over to long term, nursing care, there were a few hiccups.

Long-term resident's family · 2023★★★★

The nursing home is infested with cockroaches. This is the third or fourth time in a row that I've seen them in the hallways and the bedrooms. Some of the medical assistance were using very loud foul language in the hallway near the nursing station.

Resident's family · 2025★★★☆☆

My grandfather was in this facility for at least two months and half of the time he was in the hospital because of neglect. The staff has broken his glasses, refused to feed him since he couldn't do it himself.

Grandchild of resident · 2022☆☆☆☆

10deficiencies
Near state avg (18.9)

Autumn Lake Healthcare at Pikesville shows extensive deficiency patterns across all three surveys, with recurring problems in fire safety, medication management, and care planning that persist across multiple inspections. The facility has accumulated 99 total deficiencies spanning critical areas including repeated fire safety violations (sprinkler systems, emergency generators, smoke doors), ongoing medication errors and pharmacy oversight issues, and persistent problems with resident assessments and care plan development. While all deficiencies show correction dates, the pattern of similar violations reappearing across 2018, 2022, and 2025 surveys suggests systemic challenges with maintaining consistent compliance standards.
