The following deficiency was found during the on-site compliance inspection and investigation of complaint 00160865, conducted on March 13, 2026:
Based on observation and interview, the manager failed to ensure poisonous or toxic materials stored by the assisted living facility were maintained in labeled containers in a locked area separate from food preparation and storage, dining areas, and medications, and were inaccessible to residents. Findings include: 1. During a tour of the facility, the Compliance Officer observed no fewer than two ambulatory residents. The Compliance Officer observed a curtain separating the dining room from the facility office area, and caregiver living quarters. The Compliance Officer went through the curtain and observed a can of “Raid Flying Insect” insecticide sitting on an end table, next to a sofa. The insecticide had a label which read “HAZARDOUS TO HUMANS AND DOMESTIC ANIMALS.” Next to the sofa and end table was an open door leading to the caregiver’s living quarters. The door had a sign on it which read “Private.” The door was unlocked and open. The Compliance Officer observed the headboard of the caregiver’s bed partially blocked the doorway, preventing the door from closing and allowing for unrestricted access. The Compliance Officer went into the bedroom and bathroom. In the bathroom, the Compliance Officer observed a sink cabinet with an unsecured door. Inside the cabinet, the Compliance Officer observed numerous cleaning chemicals, including a bottle of “Clorox Toilet Bowl Cleaner.” The cleaner had a label which read “HAZARDOUS TO HUMANS AND DOMESTIC ANIMALS.” 2. In an interview, E1 advised the insecticide should not have been left unsecured. E1 removed the insecticide and secured it. E1 acknowledged the caregiver’s quarters were unsecured and allowed unrestricted access to residents. 3. In an exit interview, the findings were reviewed with E1, and no additional information was provided.
No deficiencies were found during the on-site investigation of complaint 00105025 conducted on March 18, 2025.
