An on-site investigation of complaint AZ00212819 was conducted on July 19, 2024, and the following deficiency was cited :
Based on observation and interview, the manager failed to ensure exterior doors were equipped with ramps or other devices to allow use by a resident using a wheelchair or other assistive device. The deficient practice posed a risk if a resident was unable to safely exit the facility. Findings include: 1. During the environmental inspection of the facility, the Compliance Officer observed the patio door entrance/exit to the back yard was not wheelchair accessible. There was a step from the patio to the ground level. 2. In an interview, E1 acknowledged the patio door entrance/exit did not have a ramp or other device to allow use by a resident using a wheelchair or other assistive device.
