This report details the findings of a complaint investigation conducted on 09/23/2025 regarding several allegations. All four allegations—malnutrition, lack of privacy, unsafe handling of personal items, and lack of activities—were ultimately deemed unsubstantiated based on the evidence gathered during the investigation.

The facility underwent an unannounced Case Management visit following a report of a potential resident abuse incident. The Licensing Program Analyst interviewed staff and assessed the situation, concluding that the evidence did not support any type of abuse occurring. Consequently, no deficiencies were issued for this visit.

The investigation found the allegation to be unfounded, as the resident in question was never a resident at the facility. No deficiencies were cited in this report.
