The following deficiencies were found during the on-site compliance inspection and investigation of complaint 00127229 conducted on July 8, 2025:

Based on record review and interview, for one of three residents sampled, the manager failed to obtain a documented residency agreement, which included the signature of the resident, their representative, legal guardian, or another individual designated under A.R.S. 36-3221 to make health care decisions on the resident’s behalf. Findings include: 1. A review of R2’s medical record revealed R2 was admitted in 2024 as a directed care resident. Further review revealed a residency agreement, signed by R2’s representative. However, the representative did not sign the agreement until twenty days after R2 was admitted into the facility. 2. In an interview, E1 advised the residency agreement had been emailed to the representative prior to R2’s admission date. However, evidence of the email to the representative prior to, or within five working days after, R2’s admission date was unavailable for review. 3. In an interview, E1 agreed the residency agreement was not signed by R2’s representative until twenty days after R2 moved into the facility.

Based on this deficiency-free compliance inspection, the Department shall not conduct a compliance inspection for twenty-four months, according to A.R.S. \'a7 36-425(E). Subsection (E) does not prohibit the Department from enforcing licensing requirements as authorized by A.R.S. \'a7 36-424.
