What this means for your family

While the facility is praised for its physical therapy and clean environment, the recurring reports of patient falls and neglect in the memory care unit are significant red flags. We strongly recommend that families considering this facility conduct unannounced visits and specifically inquire about their fall prevention protocols and staffing ratios during weekends and evenings.

Key Review Excerpts

My 95 yo mother broke her arm and hip and was given little to no pain meds. There was a 2 week span where she was provided nothing but Tylenol.

Medical provider/daughter · 2022☆☆☆☆

30 falls in a year and a half. A broken hip, a broken leg, and that is with family members watching my mother 6 hours per day to try to keep her safe.

Long-term resident's family · 2025☆☆☆☆

My dad was moved to this memory care unit because of the high ratings and promise of individualized care. The only thing Siena delivered was a spectacular view of Lake Michigan. Dad was there only four months and in that time he went from using a walker for balance to being wheelchair bound.
