Based on record review and staff interviews, the facility failed to provide documentation of the emergency generator's required maintenance. Failure to properly maintain and document routine emergency generator maintenance could lead to harm to residents and staff in the event of an emergency.

Based on observation, interview, and review of facility policy and procedure, the facility failed to ensure three of three sampled ice machines were cleaned and maintained as needed for sanitary food service. The deficient practice could lead to the spread of foodborne illness for residents.
