The report details two separate complaint investigations conducted on different dates (12/05/2025 and subsequent investigation on 01/26/2026). In both instances, the allegations raised regarding narcotics disposal, staff training, food supply, infection control, resident needs, and staffing levels were investigated and ultimately deemed Unfounded or Unsubstantiated.
