The following deficiencies were found during the on-site compliance inspection conducted on October 28, 2024:

A manager shall ensure that:R9-10-806.A.4.a-bCorrected Oct 31, 2024

Based on documentation review and interview, the manager failed to ensure that a caregiver's or assistant caregiver's skills and knowledge were verified and documented before the caregiver or assistant caregiver provided physical health services and according to policies and procedures. Findings include: 1. A review of facility policy and procedures, revealed a document titled "Employees and Volunteers Qualifications" which stated "The hiring individual will check and document qualification, skills and knowledge for each employee and volunteer to ensure they meet criteria and are able to perform the job duties before starting to provide assisted living services to the residents. Documentation of such check is going to be kept in the employees' records upon hiring ("Employee Orientation" and "Employee Qualification and Skills")." 2. A review of E2's personnel record revealed a document titled "Employee Orientation." However "Employee Qualification and Skills" was not included. 3. In an interview, E1 stated that E2 was providing activity of daily living support under the supervision of a licensed caregiver. E1 acknowledged that a caregiver's or assistant caregiver's skills and knowledge were not verified and documented before the caregiver or assistant caregiver provided physical health services and according to policies and procedures.

A manager shall ensure that a bell, intercom, or other mechanical means to alert employees to a resident's needs or emergencies is available and accessible in a bedroom or residential unit being uR9-10-814.ECorrected Oct 31, 2024

Based on record review, observation, and interview, a manager failed to ensure that a bell, intercom, or other mechanical means to alert employees to a resident's needs or emergencies was available and accessible in a bedroom or residential unit being used by a resident receiving personal care services. Findings include: 1. A review of R2's medical record revealed R2's service plan which documented R2 was to receive personal level of care. 2. During an environmental inspection, the Compliance Officers observed R2's bedroom without a bell, intercom, or other mechanical means to alert employees of the resident's needs. 3. During an interview, E1 reported that personnel in the home perform rounds every 15 minutes confirming the needs of residents. E1 acknowledged residents did not have a bell, intercom, or other mechanical means to alert employees to a resident's needs or emergencies available and accessible in resident bedrooms.

Based on observation and interview, the manager failed to ensure at least one bathroom was accessible from a common area and contained a window that opens or another means of ventilation. Findings include: 1. During an environmental inspection, the Compliance Officers observed two bathrooms accessible from the common area. Both bathrooms had one switch, which was suppose to operate the light and fan. However, when the Compliance Officers turned the light on, the fan did not turn on in either bathroom. Neither bathroom had another means of ventilation. 2. During an interview, E1 acknowledged the manager failed to ensure at least one bathroom was accessible from a common area and contained a window that opens or another means of ventilation.
