What this means for your family

Applewood features a strong therapy department and a clean, modern environment that many families appreciate. However, because multiple reviewers have reported critical lapses in hygiene, call-light response times, and communication, you must be physically present to advocate for your loved one. I strongly recommend visiting during off-hours and weekends to see if staffing levels meet your expectations before committing.

Questions for Your Tour

  • 1Given the current staffing levels, what is your average response time for call lights, and how do you prioritize these requests when the floor is busy?
  • 2Can you walk me through your process for ensuring residents receive consistent assistance with daily hygiene and toileting, especially during peak morning hours?
  • 3What is your protocol for keeping families updated on medical changes or health status, and how do you ensure this communication is consistent?
  • 4How do you manage dietary requirements for residents with specific health needs like diabetes, and how is this monitored during meal service?
  • 5What measures are in place to track and secure residents' personal belongings and clothing to prevent loss?
  • 6Since I noticed your administrative team is quite active in responding to feedback, what is the best way for families to reach out if they have concerns about their loved one's care?

Key Review Excerpts

My mother was blind, they had her in isolation. There are no phones bedside. They checked on my mother maybe every 4 to 5 hrs. they put her in depends and would not take her to the bathroom.

Long-term resident's family · 2022☆☆☆☆

My dad has been in long term care at Applewood for the past few months. We couldn't be happier! He receives excellent care from all the staff. This is our 4th facility in the past 9 months and trust me, there are some REALLY bad places out there...this is not one of them.

Long-term resident's family · 2022★★★★★

The facility isn’t as clean as you would expect it to be, patients eat in the hallways, rooms are small and uncomfortable, if you want a TV in your room it will be 2 feet from the bed. Don’t let them do the laundry for your family member either, they lose clothes.

Applewood Nursing Center shows a concerning pattern of recurring deficiencies across multiple surveys, with families filing complaints that triggered 18 citations. The facility has persistent problems with nutrition and meal quality, safety hazards and accident prevention, and fire safety systems. While all deficiencies show correction dates, the same issues repeatedly resurface - particularly food safety violations, accident hazards, and pressure ulcer care problems that have appeared across multiple years, suggesting ongoing systemic challenges with maintaining consistent care standards.
