What this means for your family

This facility is an excellent choice for patients needing intensive physical or occupational therapy, as the rehab teams are frequently cited as 'first class.' However, families should closely monitor medication schedules and be prepared to advocate for more frequent assistance during night shifts, as some reviewers noted delays in response times.

Key Review Excerpts

The OT and PT staff are great as are the CNAs and nurses

Rehab patient · 2026★★★★

My 80 year old brother was in Apple Valley for 2 weeks in May after a stroke that affected his strength, balance and speech. Apple Valley staff were excellent in their care of my brother and the physical, occupational and speech therapists that he saw almost daily were first class in bringing him back to where he could walk with a walker and speak much more clearly.

Rehab patient's family · 2025★★★★

The staff was very hospitable, SMILING and greeting my husband and myself.

This facility shows concerning patterns with six family complaints triggering federal investigations, including recent issues with abuse reporting and accident prevention. The most recurring problems involve safety hazards and accident prevention, resident rights violations, and fire safety deficiencies. While all deficiencies show correction dates, the repeated safety issues across multiple surveys and ongoing complaints suggest persistent care quality challenges that families should carefully evaluate during visits.
