Specifically, there was no documentation of an HCPR check for Staff B, a Personal Care Aide. The Administrator stated she believed HCPR checks were only required for Certified Nursing Assistants.

The facility failed to ensure that all staff members had a Health Care Personnel Registry check completed upon hire. Specifically, personnel files for two staff members lacked documentation of these required registry checks.

The facility failed to ensure that two of three sampled staff members had a Health Care Personnel Registry check performed upon hire. Personnel files for Staff B and Staff C lacked documentation of these required registry checks.

The facility failed to ensure that two of three sampled staff members had received statewide criminal background checks. Staff B and Staff C had only received county-wide criminal background checks since being hired at the facility.

The facility failed to maintain clean and well-repaired walls, ceilings, and floors. Specific issues included torn vinyl flooring in resident bedrooms, damaged window blinds, a loose and dirty ceiling air vent, and various scuff marks and grime buildup throughout the home.
