Based on documentation review, record review, and interview, the manager failed to ensure documentation was maintained of the caregivers and assistant caregivers working each day, including the hours worked by each. The deficient practice posed a risk if there was no record to ensure shifts and tasks were covered. Findings include: 1. A review of the facility's policies and procedures (dated April 1, 2023) revealed a policy titled "Personnel Requirements (Staffing)." The policy stated "C. The manager shall ensure that a personnel schedule: 1. Indicate the date, scheduled work hours and name of each Employee assigned; 2. Reflects actual work hours; and 3. Is maintained on the premises for at least 12 months from the last date on the schedule." 2. The Compliance Officer observed E4 on the premises and working when the Compliance Officer arrived at approximately 9:00 AM. 3. The Compliance Officer observed a document posted titled "Work Schedule" dated August 13-26, 2023. However, E4 was not documented on the "Work Schedule" including the hours worked. 4. A review of E4's personnel record revealed E4 was hired as an assistant caregiver in May 2023. 5. A review of facility documentation revealed documentation of the caregivers and assistant caregivers working each day, including the hours worked by each; dated from May 2023 to August 2023. However, E4 was not included on this documentation. 6. In a joint interview, E1 and E2 acknowledged documentation was not maintained of the caregivers and assistant caregivers working each day, including the hours worked by each.
