The facility failed to ensure that all outside entrance/exit doors had a continuously sounding device activated when opened for a resident with dementia and disorientation. Specifically, the front and back doors utilized alarms that only provided a single verbal announcement rather than a continuous sound. This deficiency was noted during observations of resident behavior and door usage.
