Questions for Your Tour

  • 1Given the importance of consistent personal care, what specific protocols are in place to ensure residents receive timely assistance with hygiene and toileting needs?
  • 2I noticed your staffing rating is lower than your overall rating; how do you manage daily care tasks to ensure residents aren't waiting too long for assistance?
  • 3I see that you actively engage with families through online feedback; how do you incorporate that family input into improving your communication regarding a resident's health status?
  • 4Your rehabilitation team comes highly recommended, so how do you coordinate their efforts with your nursing staff to ensure medication management remains accurate and safe?

Key Review Excerpts

My grandmother has been bounced to this place after her surgery, and these people didn't bother to check or change her wound dressing for TWO WEEKS. She didn't receive a bath or shower or even an option for one, refused to contact her physicians even when she had fever and chills for over 24 hours.

Memory care family member · 2025☆☆☆☆

I love this place. The staff is always friendly and quick to answer any concerns and questions. The nurses in the dementia area are top rate! Susan Conkle is always ready to help me when I have questions.

Long-term resident's family · 2025★★★★★

There is no reason someone should have to sit in there own bowel movement from breakfast to passed lunch. After calling for a nurse to come help. This place has done nothing for my grandma. 4 hours with out something to drink etc.

This facility shows recurring problems with food safety and fire safety systems across all three surveys, suggesting persistent operational challenges. The most recent December 2024 inspection revealed extensive deficiencies in resident care quality, nutrition services, and resident rights protections. Families have filed complaints about pressure ulcer care, indicating potential concerns about wound management. While the facility has correction plans for all violations, the pattern of repeated food safety violations and the significant scope of recent deficiencies warrant careful consideration before placement.
