FACILITY EVALUATION REPORT

The facility underwent an unannounced Case Management Health and Safety visit on March 6, 2026. The Licensing Program Analyst observed that facility utilities were operating, and food supplies exceeded the required minimums. No deficiencies were observed or cited during this visit, indicating no immediate threats to resident health or safety.

The unannounced Case Management Health and Safety visit on December 9, 2025, found no immediate health or safety issues. The facility was observed to have adequate food supplies, and medications were properly secured. No deficiencies were cited during this inspection.

The unannounced Case Management Health and Safety visit on October 31, 2025, found no immediate health or safety issues observed at the facility. The LPA noted that the facility's food supply exceeds required minimums, and medications were properly secured and inaccessible to residents.

The facility underwent an unannounced Case Management Health and Safety visit on August 26, 2025. The inspector noted that no immediate health or safety issues were observed during the tour or record review. Specifically, food supplies exceeded required minimums, and medications were properly secured.

The facility was inspected for its required annual review and was generally observed to be clean and clutter-free. The primary deficiency noted was the failure to maintain staff with current CPR certification, which constitutes a Type A violation due to the immediate risk posed to residents. The facility was advised on upcoming fees and required follow-up actions.

This report details the findings of a complaint investigation conducted on 01/28/2025 regarding allegations of staff withholding medical records and taking personal property. Both allegations were deemed UNSUBSTANTIATED due to a lack of evidence to prove the claims.
