The facility failed to ensure the coordination of health care for a resident regarding wound care. Specifically, the facility could not perform required wet-to-dry dressing changes and failed to properly follow up on orders to meet the resident's acute health care needs.

Additionally, medications were found left in residents' rooms, and there was an inconsistency between insulin orders and the EMAR due to a misunderstanding of unit versus solution amounts.

The facility failed to ensure referral and follow-up to meet the acute health care needs of a resident by failing to inform a primary care provider of skin breakdown. Specifically, the facility did not notify the physician regarding a wound for Resident #2.
