FACILITY EVALUATION REPORT

The facility underwent a required 1-year annual inspection. The inspector observed that the facility maintained good standards regarding safety, supplies, and resident engagement. No deficiencies were cited during this inspection.

The inspection was conducted on 07/15/2025 as a case management visit to gather information regarding a death report from 07/14/2025. The Licensing Program Analyst reviewed the resident's records and obtained necessary documentation. No deficiencies were cited during this visit.

The facility underwent a Required-1 Year inspection and was found to have one critical deficiency related to medication administration. Specifically, the facility failed to provide medication as ordered on two separate dates, which constitutes an immediate health risk. An immediate civil penalty of $1000.00 was issued due to this violation.

The inspection was conducted as a Case Management follow-up regarding medication discrepancies. Two incidents were reviewed: one concerning Amoxicillin dosage and another involving an unknown capsule (Tamiflu). A civil penalty was issued for a repeat violation of Regulation 87465(a)(4).

The inspection was a Case Management visit following multiple incident reports. Significant deficiencies were noted regarding medication administration, including failure to assist with self-administered medications and failure to ensure prescribed doses were given. The report also notes this is a repeated violation, resulting in an immediate civil penalty.
