A complaint survey, prompted by #CO36733, #CO36804, #CO36807 and #CO36870 was conducted on 7/22/24 to 7/29/24. Twelve deficiences were cited. Based on interviews and record review, the facility failed to ensure an effective quality assurance program to identify and address facility compliance concerns was implemented, in order to facilitate improvement in the lives of nursing..
